Below are the documents needed for your client appointments. For each client appointment you will complete the appropriate paperwork and submit to your Mentor within 24 hours of the appointment. This will ensure you are completing all necessary assessments and help guide the flow of your appointments. Timely and accurate documentation is a foundational piece in starting your professional practice.
In the beginning, we will map out the basics together and then you will submit your final drafts. As we focus on whats important to you, little by little your vision will become more clear. As you gain experiences with clients and in our private sessions, you will learn the necessary steps to help your clients and make the process easier for yourself. You may adapt consents and charting documents as necessary to meet the needs of your private practice. Having clear paperwork is a must in order to protect yourself legally. Do not perform any appointments without paperwork!
